Rapid i.v. injection of emulsified Vit K produced flushing, breathlessness, a sense of constriction in the chest, fall in BP, few deaths are on record.

The only use of Vit K is in prophylaxis and treatment of bleeding due to deficiency of clotting factors in the following situations:

Contra Indication

Caution

Manadione and its water soluble derivatives can cause haemolysis in patients with G6PD deficiency and neonates. It is not advised to use Menadione because of poor efficacy and higher toxicity.

Side Effect

Co-administration with thiazide diuretics or Vit D may lead to milk alkali syndrome and hypercalcaemia. Decreased absorption with coricosteroids. Decreases absorption of tetracyclinea, atenolol, iron, quinolones, alendronate, Na fluoride, Zn and calcium-channel blockers. Enhances cardiac effects of digitialis glycosides and may precipitate digitalis intoxication.

Calcium supplements are usually required where dietary calcium intake is deficient. This dietary requirement varies with age and is relatively greater in childhood, pregnancy and lactation due to an increased demand.

Contra Indication

Patients with Calculi renal calculi or history of renal calculi; hypercalcaemia; hypophosphataemia.

Caution

Renal impairment, hypercalcaemia-associated disease eg. other malignancies; sarcoidosis; elderly.

Renal impairment, monitor serum levels especially in patients with renal failure, hypercalciuria.
